- comments: no city of chicago certified food service manager on site while foods are being prepared for customers. Shrimp and other sea foods. Instructed that a city of chicago food service manager must be on site at all times while facility is open and operating. Priority foundation violations 7-38-012 citation issued
49. Non-food/food contact surfaces clean
- comments: observed the walk in cooling unit floor and the walk in freezer floor both with a build up of debris and dirt on floor and ice building up on the floor of the walk in freezer. Instructed to clean and maintain both units.

- comments: observed the sides of the deep fryers with a havy build up of grease and food debris. Observed the area under the grill and the flat top with a build up of dirt, dust and small debris. Instructed to clean and maintain.

- comments: observed food splatter on the wall behind the cooking equipment and a build up of grease and small debris on the floor. Instructed to clean the wall behind the cooking equipment and the floor under and around the cooking equipment.

- comments: observed contiued non compliance from report #2604136 dated 10/04/2024 with a correct by date of 12/26/2024 4-301. 14: 2 burner stove not under ventilation system. Instd to have completely under ventilation hood or remove from premises. Priority foundation violation. Citation issued 7-42-090
