- comments: observed small holes at the bottom of the front entrance door; instructed to seal holes to avoid possible rodent entry to premises.

- comments: observed a live cat on premises. Cat was seen in the rear storage area of the store along with food, water and a litter box. Instructed that no live animals are allowed in a retail food establishment; must remove cat from premises. Priority foundation violation 7-40-060 issued.

- comments: observed evidence of rodents on premises. Found 100 mouse droppings on the floor of an unused walk-in cooler. Instructed to remove all droppings from the floor of the cooler. Priority foundaton violation 7-38-020(a) issued.

- comments: instructed to clean the interiors of the exposed handsink and three comp sink in the rear areas. Some unused equipment (display case and coolers) need cleaning as well.

- comments: observed some plumbing not in good repair according to law. Found the rear of the faucet at the utility sink not connected to the hot water line and hot water cannot flow through the faucet neck as required; must correct. Also observed low water pressure when hot water is running at both washroom sinks; must correct so that both sinks have adequate hot running water pressure. Priority violation 7-38-030(c) issued.

- comments: observed no self closing devices at washroom doors; must install at both doors.

- comments: observed peeling paint on the ceiling of the walk-in cooler that is working; must scrape peeling paint from cooler ceiling. Observed several areas in the store with damaged floor tile; must replace or repair floor tile where needed. Observed several unfinished walls (dry wall); instructed to finish walls by painting or sealing to make smooth and easily cleanable.

- comments: observed storage rooms cluttered with boxes and unused equipment; instructed to remove items and keep areas free of clutter to avoid possible pest harborage.

- comments: instructed to clean floors throughout establishment including washrooms, storage rooms and walk-in units. Clean ceiling vent fans in washrooms to remove dust observed and wall in womens washroom to remove spider webs.
