- comments: observed approximately 40 small rodent droppings throughout the facility in the following places during the time of inspection. Along the side of stove on the floor and behind refrigerator 1 in prep space, in the dry storage pantry on lower shelves and on the floor in corners of said room, in the room where freezer is located in the rear facility, on the floor scattered around the hot water tank as well as utility sink room. Instructed to remove all said droppings and clean and sanitize affected areas. Must eliminate activity on premise. Priority foundation violation. #7-38-020 (a). Citation issued.

- comments: observed children in hands washing sink is to far right in classroom in rear facility with leaky piping and water pooing onto floor during time of inspection. Instructed to repair and maintain in good condition.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: instructed to provide a closed garbage can for employee toilet room.
55. Physical facilities installed, maintained & clean
- comments: observed food debris on floors throughout kitchen area, and space where freezer is located. Instructed to clean and maintain.
