- comments: observed no valid original city of chicago certified food manager certificate holder on premise during time of inspection while preparing and serving burger patties, green peas, and milk. Instructed to provide at least one and maintain on site at all times. Priority foundation violation. #7-38-012. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on the premise during the time of inspection. Noted approximately 5 small live black flies and 15 small rodent droppings on the side of the long counter near the exit door to the boiler room and 2 on the side of the stainless-steel cooling unit in the rear prep area. Noted 3 large live spiders along the wall in the first classroom upon entry to the premises. Recommended to contact pest control for service and eliminate pest activity. Priority foundation violation #-38-020 (a). Citation issued.

- comments: observed dead insects (spiders) and cobwebs throughout all windowsills and in corners of both classrooms in play areas. Instructed to remove.
51. Plumbing installed; proper backflow devices
- comments: observed toilet room on 2nd floor across from unused classroom hot water nozzle in disrepair. Instructed to repair and maintain in good condition.
55. Physical facilities installed, maintained & clean
- comments: observed 2 storage closets in paul robeson dr classroom as well as pantry/storage closet with excessive clutter. Instructed to organize all items, remove unnecesary and elevate at least 6 inches off floor.
56. Adequate ventilation & lighting; designated areas used
- comments: observed inadequate lighting throughout prep kitchen area. Instructed to increase.
