- comments: observed no city of chicago food service sanitation manager on duty while time and temperature control for safety (tcs) foods are stored, handled and served. Instructed that a certified food manager must be on site during tcs food prep/handling, serving and storage during operating hours. Priority foundation violation. 7-38-012. Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located in the prep area with a build-up of dirt and pink slime substance at the metal interior of the ice machine and over the ice cubes stored below in ice bin. Ice is used for human consumption. Instructed to remove ice, clean and sanitize. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed food storage containers without labels containing the common name of foods. Instructed to provide.
39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored improperly on the floor in walk-in cooler. Instructed to stored properly by elevating at least 6 inches from the floor on raised shelving units. Observed food items stored in prep and walk-in coolers uncovered. Instructed to provided food grade lids and/or coverings.
43. In-use utensils: properly stored
- comments: observed in-use utensils stored improperly directly inside of food container(s). Instructed to properly store all utensils between usage.
49. Non-food/food contact surfaces clean
- comments: observed grease buildup on cooking equipment, walls, surrounding floors and shelving units in the kitchen/prep areas. Instructed to clean.
51. Plumbing installed; proper backflow devices
- comments: observed drainpipe leaking under hand washing sink in the dish wash area. Instructed to repair.
55. Physical facilities installed, maintained & clean
- comments: observed mop stored improperly in a position that does not allow to dry properly. Instructed to store mop properly.

- comments: observed an accumulation of dust/dirt/debris/grease in ventiliation over the stove/grill/fry area. Instructed to clean.
