1. Person in charge present, demonstrates knowledge, and performs duties
- comments: person in charge does not have a city of chicago sanitation certificate. Priority foundation violation.

2. City of chicago food service sanitation certificate
- comments: found no certified food manager on site while tcs foods (beef, chicken, pork, soups etc) are being prepared and served. Instructed person in charge, a certified food manager with city of chicago foodservice sanitation certificate must be present at all times when handling tcs foods. Priority foundation violation 7-38-012 citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: instructed to provide handwashing signage at handsink behind the bar.

Noted box of shrimp and sauce container stored at the said handsink unable for employees to wash hands in main prep/cooking area. Handsink in the basement not accessible with cleaning supplies stored in it and handsink in the bar area not accessible with multi use utensils stored. Instructed person in charge, a handwashing sink must be accessible for handwashing at all times. (cos corrected on site - person in charge removed items from the said handsinks) priority foundation violation 7-38-030 (c) citation issued

- comments: observed the following prepared tcs foods not properly date labeled with an expiration date no longer than 7 days. Cooked tripe,cooked beef, refried beans, ground beef, cooked chicken, beef tongue stored inside reach-in coolers and prep coolers. Person in charge discarded all ready to eat food products. Total weight 50 lbs total value $ 1000,00. Instructed person in charge to properly label and date all ready to eat tcs foods held over 24 hours inside coolers. Must identify product shelf life or 'consume by' dating (7 days from preparation date). Priority foundation violation 7-38-005 citation issued

- comments: found incomplete consumer advisory disclosure and reminder statement on the menu. Management instructed to disclose which foods can be ordered 'raw' or under cooked and link those foods with an asterisk (*) to a reminder statement that states that eating raw or under cooked foods can increase your risk of food borne illness. Priority foundation violation 7-38-005

- comments: instructed to provide thermometer visible inside all coolers.

- comments: instructed to detail clean and maintain interior surfaces of all prep cooler, reach-in coolers, walk-in coolers and freezers.

- comments: backflow preventer device not located behind (2) ice machine. Instructed to install so that it can be located to be serviced and maintained.

- comments: instructed to detail clean and maintain floors along the walls and in all corners throughout prep/cooking area, under and around 3 compartment sink, under and around cooking equipment and floors throughout basement prep area.

- comments: instructed to replace non working (2) lightbulbs inside 3 door reach-in cooler in prep/cooking area.
