- comments: observed flour, seasioning and sugar out of the original containers without labels containing the product name. Instructed to label all foods and spices with common name that are out of the original containers.
39. Contamination prevented during food preparation, storage & display
- comments: observed to go containers stored facing up on shelf in prep area. Instructed to invert all equipment to prevent contamination.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed the opening and under the deep fryers with a build up of grease and food debris. Instructed to clean and maintain.
49. Non-food/food contact surfaces clean
- comments: must detail clean the exterior of all cooking equipment to remove grease and old food debris. Also detail clean the interior of all coolers, prep tables, counter tops and exterior of spice storage containers.

- comments: observed an excessive amount of debris and ice buildup on the condenser of the walk in freezer. Must remove excess ice, detail clean, and maintain.

- comments: observed the side of all the cooking equipment and non cooking equipment (all shelving across from the hot line side )with a heavy build up of grease and food debris. Instructed to clean and maintain all areas.

- comments: instructed manager to detail clean all walls, ceilings and attachment (pipe,cords,beams and vent cover) through out premises to remove grease, dust and debris.

- comments: observed floors dirty and in poor repair in spots. Must repair broken/damaged floors to prevent water from puddling and creating a breeding place for flies. Also detail clean to remove spills, grease and debris though out.

- comments: observed soiled mop heads not properly stored. Instructed manager to hang mop heads to prevent insect breeding.

- comments: observed one burnt out light bulb in the meat department. Instructed to replace.

- comments: observed no allergen training on site at time of inspection. Instructed all employees holding city of chicago foodservice manager certificates must complete allergen training and maintain records thereof on site at all times.
