Report date: Aug 9, 20225. Procedures for responding to vomiting and diarrheal events
- comments: no vomit and diarrheal clean up procedure on premises as required; must provide. Instructed that a vomit and diarrheal clean up procedure must be documented and the necessary supplies on hand in the event of a vomit and diarrheal incident on premises. Priority foundation violation 7-38-005. Citation issued.
40. Personal cleanliness
- comments: observed no hair restraints on employee while in food prep area. All employees must wear hair restraints while in food prep area. Provided during inspection.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: found duct tape used as a form of repair for ice chest in front prep area. Must remove tape and repair, maintain.
49. Non-food/food contact surfaces clean
- comments: found excessive ice accumulation inside chest freezer in front of prep area. Instructed to defrost, clean and sanitize. Must maintain same.

- comments: found cardboard lined on shelves in dry storage area, walk in cooler, and prep area. Must remove in order to make non-porous, smooth and easily cleanable.

- comments: found clear liquid on floor in utiliy room. Must clean and maintain a dry floor.
57. All food employees have food handler training
- comments: instructed must provide proof of required food handler training for all employees.
60. Previous core violation corrected
- comments: observed previously issued core violations not corrected from report #2537654, dated 11/2/21. Must correct the following violations: #49 instructed to clean the interior bottom of the fryers to remove grease, clean underneath the grill to remove grease, clean shelves under front counter. #55 instructed to clean the floors around under and behind kitchen equipment and freezer units, clean walls around and behind equipment where needed. Clean interiors of light shields to remove insects the storage closet where the hot water tank is located is cluttered; instructed to remove unnecessary items to avoid clutter and possible pest harborage. #58 instructed that all city of chicago food service managers must obtain the required food allergen training. Priority foundation violation 7-42-090. Citation issued.
