- comments: observed inside the bottom of the deep fryer units with a build up of grease and food debris. Instructed to clean and maintain.

- comments: observed the sides on the cooking and non cooking equipment with a build up of dust, grease and some food splatter. Instructed to clean and maintain as a means of preventing possible pest and insect harboring.
55. Physical facilities installed, maintained & clean
- comments: observed the floors in the facility under and around equipment with a build up of dirt and grease. Observed the mop sink area with a build up of dirt and standing water. Instructed to clean and maintain as a means of preventing insect harboring.
56. Adequate ventilation & lighting; designated areas used
- comments: observed a build up of dust on the outside of the hood ventilation system and on the outside. Instructed to clean and maintain as a means of preventing possible contamination of foods being prepared and or cooked.
