- comments: 6-201. 17: observed a missing ceiling tile above the walkway in the serving area, near manager office. Ceiling observed water damaged and floor with water stains from a previous leak. Instructed to repair leak and replace ceiling time.

- comments: 8-404. 13 (b:3) observed continued non-compliance not corrected as follows: 38: still noted observed approximately 25 or more mice droppings on site during inspection. Found the in the following areas on the floor: under the shelves in the food prep area, on the floor under the dishware storage cages in the culinary room, and on the windowsills behind the storage cages in the culinary room. Instructed to remove droppings, clean affected areas, and maintain. Priority violation 7-42-090.
