14. Required records available: shellstock tags, parasite destruction
- comments: the date when the last shellstock was sold is not recorded on all shellstock tags that were provided on site. The facility must record the date when the last shellstock was sold. Facility manager went through all invoice receipts in house to verify when last shellstock items were sold per tag. Priority foundation violation 7-38-005 citation issued.

38. Insects, rodents, & animals not present
- comments: insect control device is over food prep area in 1st floor bakery department and 2nd floor deli prep department. Devices shall be installed so that: 1)they are not over food preparation areas, 2) dead insects or insect parts do not fall onto exposed food, equipment, utensils, or single service items. Manager reloacted device.

39. Contamination prevented during food preparation, storage & display
- comments: containers of food stored on the floor in the sushi walk-in cooler and sushi front prep-station. Items must be placed 6 inches above the floor.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: milk and wood crates used as elevation for storage in the following areas; cheese department, sushi prep, sushi walk-in cooler, smokehouse department, and 2nd floor bakery. Facility must remove and replace with shelving that is smooth, easily cleanable, durable, non-absorbent, and that is easy to monitor debris behind/underneath.

- comments: observed mini fan unit in donut display case soiled with debris. Facility must clean and maintain.

- comments: observed 2-door reach-in cooler and gaskets soiled with food debris in smokehouse department. Facility must clean and maintian.

55. Physical facilities installed, maintained & clean
- comments: unnecessary items / nonfunctional equipment found in storage area behind cheese department (deli slicers). The premises shall be free of items unnecessary to the operation or maintenance of the establishment. This includes equipment that is nonfunctional or no longer used.

56. Adequate ventilation & lighting; designated areas used
- comments: employee clothing items were found stored improperly in shushi prep-room on dry storage shelves. Food establishments shall a) provide designated dressing rooms or dressing areas for employees that routinely change their clothes in the establishments; b) provide lockers or other suitable facilities for the orderly storage of employees' clothing and other possessions.
