- comments: noted cooked steak and tomato sauce in walk-in cooler with white mold. Approximately 2 lbs at $20. Person in charge discarded and denatured product. Priority violation 7-40-050(a) citation issued

16. Food-contact surfaces: cleaned & sanitized
- comments: observed black, pink and brown slime substance dripping onto ice cubes stored at below ice bin in rear kitchen/dish area ice machine. Ice is used for human consumption. Instructed to remove ice deep clean and sanitize and maintain. Priority foundation violation #7-38-005. Citation issued

37. Food properly labeled; original container
- comments: noted dry ingredients at all prep tabels and cooks line without labels. Instructed to provide labels of common names for items removed from its original container.

40. Personal cleanliness
- comments: noted food handler in kitchen and throughout prep area without proper hair restraints. Instructed to provide.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed rice water, sopa, dry chiles, and salsa stored in 5 gallon homedepot and menards buckets in walking cooler. Management instructed to use proper food grade storage bags and containers only.
