- comments: observed no hand washing signs at hand washing sinks in prep, dish washing and washroom areas. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed over 20 rat droppings on floor along washing machine in rear storage area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed openings along exterior of building and several burrows along perimenter of building near rear exit door. Instructed manager to seal all outer openings to prevent pest entry.

- comments: observed happy meal boxes stored on floor in rear storage area. Instructed manager to elevate all items six inches off floor.

- comments: observed accumulated grease, food debris and spills on interior and exterior surfaces of fryers, holding units, grill, coolers, freezers, shelving units, beverage machines. Instructed manager to clean and maintain.

- comments: observed liquid waste on ground around large dumpster. Instructed manager to clean and maintain outside garbage area.

- comments: observed missing ceiling tiles, openings around electrical outlets, damaged floor under pop machine, missing grout on floor tiles in drive thru area, openings in floor near washing machine and along baseboards in prep and storage areas. Instructed manager to seal all openings in all areas and maintain.
