Report date: Jun 1, 20235. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit clean-up kit on site at the time of inspection. Left one pager and instructed to maintain procedure and kit containing all necessary items, including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. Citation issued.
9. No bare hand contact with rte food or a pre-approved alternative procedure properly allowed
- comments: observed a female cook take a breakfast burrito from the line and wrap it in paper with bare hands. Management discarded approximately 1lb of product valued at approximately $10. Instructed all food handlers must use gloves when handling ready-to-eat foods. Priority violation 7-38-010. Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed build-up of black and pink slime in the interior of ice machine in the basement used to produce ice for human consumption. Instructed to remove ice, clean, sanitize, and maintain unit free of any slime. Priority foundation violation 7-38-005. Citation issued.

- comments: observed bulk container of flour in rear not labeled. Instructed to correct.

- comments: observed aluminium foil used to line prep table and side of storage rack. Instructed to remove aluminium foil and maintain smooth, cleanable surfaces.

- comments: observed heavy build-up of dirt, grime, and food debris on walls on the cook line, in the dish area, and in the utility sink. Instructed to detail clean and maintain in good condition.

- comments: observed build-up of grease, grime, food debris, and dust on the hood over the cook line. Instructed to detail clean and maintain in good condition.
58. Allergen training as required
- comments: observed no allergen training certificates on site at the time of inspection. Instructed all employees holding city of chicago foodservice manager certificate must complete allergen training and maintain records thereof on site at all times.
61. Summary report displayed and visible to the public
- comments: observed no summary posted for public view from previous inspection #2546137 dated 1/5/2022. Left new summary and instructed to post for public view. Priority foundation violation 7-42-010(b). Citation issued.
