- comments: observed person in charge no valid proof of food safety management training during time of inspection. Must provide.
2. City of chicago food service sanitation certificate
- comments: observed facility food handlers preparing and handling time and temperature control for safety (tcs) foods (empanadas and creamed pastries) without a city of chicago food service manager certificate holder on site. Informed person in charge that a certified manager with an original city of chicago food service certificate must be on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.
6. Proper eating, tasting, drinking, or tobacco use
- comments: noted employee drinks & cups on front prep table. Instructed to store and consume drinks in designated area.
23. Proper date marking and disposition
- comments: observed ready-to-eat, time/temperature control for safety (tcs) foods (prepared whipped cream, horchata, blueberry filling) in kitchen 2 door refrigerator not labeled to indicate the product's name and date by which the food must be sold, discarded, or consumed within 7 days of preparation. Instructed to provide proper labels for date marking and discard date for refrigerated, ready-to-eat, tcs foods held over 24 hours placed in all cold-holding units. Person in charge discarded and denatured approx. 15 lbs valued at $20 priority violation #7-38-005. Citation issued
37. Food properly labeled; original container
- comments: noted dry ingredients in kitchen on shelves and tabletops stored in containers with out labels. Instructed to provide labels of common names for all items removed from its original container and maintain.
39. Contamination prevented during food preparation, storage & display
- comments: observed ready to eat pastries: alfajor, guava cream cheese pop tart, chocolate chip cookie, mixed berry pop tart, banana nut muffin, blueberry muffin, smores brownie, and donut countertop display shelves not protected. Instructed to wrap and label with product name and ingredients, or provide proper display case to protect food from the public. Person in charge discarded and denatured approximately 20 lbs, valued at $75. Priority violation #7-38-005. Citation issued.
40. Personal cleanliness
- comments: noted food handlers without proper hair restraints. Instructed to provide hair covers and maintain.
41. Wiping cloths: properly used & stored
- comments: noted wiping cloths draped over front prep area handsink and 3 compartment sink and on prep tabletops. Instructed to hold wiping cloths in proper sanitizing buckets in between use to prevent contamination on food prep surfaces.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit for quaternary sanitizer used at 3 3-compartment sink. I informed the person in charge that a test kit that accurately measures the concentration of sanitizing solution must be provided. Priority foundation violation #7-38-005. Citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed clutter throughout basement. Items stored in plastic bags and boxes on the floor. Instructed to remove all unnecessary items. All kept items are to be stored on proper shelving that is non-porous, smooth and easy to clean and a minimum of 6-inches off the floor.
57. All food employees have food handler training
- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.
58. Allergen training as required
- comments: observed no certified food manager with allergen training. Instructed any persons with the city of chicago food sanitation certificate to comply with completion of training. Must provide documentation.
