- comments: observed slight calcium build up inside ice machine. Must clean interior of ice machine and maintain.

37. Food properly labeled; original container
- comments: observed some prep area bulk containers not labeled. Instructed manager must label all bulk containers.

38. Insects, rodents, & animals not present
- comments: observed 2 live roaches in 2-door reach-in freezer rubber door gaskets. Must remove all roach activity by the date of reinspection. Advised to call pest control operator for service. Priority foundation 7-38-020(a).

49. Non-food/food contact surfaces clean
- comments: observed accumulated food debris on the exterior/interior of prep area coolers and prep area shelves. Must clean equipment and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed accumulated food and dirt build-up on floor in prep and dishwashing area. Must detail clean floor throughout prep and dishwashing area.

58. Allergen training as required
- comments: no food allergen training certificate on premises. Instructed all city of chicago food managers must have food allergen training certificates.
