- comments: observed facility without a valid city of chicago food service sanitation certificate posted onsite for food employees. Informed person in charge city of chicago original food service certificate must be posted along with the certified manager on site when time and temperature control for safety(tcs) foods are stored. Priority foundation violation #7-38-012. Citation issued.

- comments: observed no hand soap available at handsink in prep/diswashing area. Instructed to provide handsoap at all times. Priority foundation violation 7-38-030 (c) citation issued

- comments: observed no paper towels at exposed hand washing sink in prep/dish area. Instructed to maintain paper towels at all exposed hand washing sinks at all times. Priority foundation violation 7-38-030(c). Citation combined with above violation.

- comments: inadequate pests control, found several live roaches through out dish room/ prep. Instructed to eliminate insects activity in all affected areas, pest control service is recommended. Priority foundation violation 7-38-020(a)

- comments: must not use duct tape as a means of repair on chest freezer. The surface must be smooth and cleanable

- comments: found the three compartment sink not maintained; sink is slow draining and clocked. Instructed management to get drainage system serviced. Priority foundation 7-38-025. Citation issued.

- comments: observed accumulated food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, shelving units. Instructed manager to clean and maintain.

- comments: observed debris on floors along walls and corners in utility and storage areas. Instructed manager to clean and maintain.

- comments: observed build up of dust on fan in both employee toilet rooms located in basementprep/dish area. Instructed to clean and maintain in good condition.

- comments: observed no food handler certificates on site at time of inspection. Instructed all employees must complete food handler training and maintain records thereof on site at all times.
