- comments: 4-102. 11 observed soda crates being used to store items on top of the rear food prep area. Instructed to provide proper shelving that is at least six inches from the floor to make the area easy to clean and maintain.

- comments: 5-205. 15 observed a leak at the bottom of the three-compartment sink in the rear food prep area. Instructed to repair and maintain in good working order

- comments: 6-201. 16 observed two badly stained ceiling tiles over the reach in beverage cooler falling in. Instructed to repair and maintain.

- comments: 6-501. 12 observed a build up of dust, dirt and debris under and behind equipment and shelving throughout the facility. Instructed to clean and maintain all areas as needed.

- comments: 8-404. 13 (b:3) observed continued non-compliance from report #2609929 on january 2, 2025. Violation is as follows. . Observed approx 15-20 mouse droppings in the rear food prep area under the soda crates near the rear exit door. Observed another set of droppings approx 10-15 in the toilet room behind the toilet and also observed another 15-20 droppings on the shelving where the cat and dog food is stored in the rear right side of the store. Also observed 3 live roaches, two at the three comp sinks, behind the right water knob, and one just over the sink at the ceiling edge. Instructed to clean and maintain area and have pest control service with proof of service sheet before return inspection of january 9, 2025. Priority foundation violation 7-38-020 (a) citation issued. Observed an additional 30 small rodent droppings in front of store scattered on prep table as well as 3 live mice running in prep area under 3 compartment sink and shelving units in grocery area. Instructed to correct. Priority violation. #7-42-090. Citation issued.
