- comments: observed windowsills in the dining area with dead insects throughout. Instructed to remove.
38. Insects, rodents, & animals not present
- comments: observed live insect activity on the premises during the time of inspection. Observed 7 live small flies on the walls and ceiling in dishwashing area. Observed approx. 20 live small flies were observed on the walls and flying around the unused dishwashing machine stored in the hallway connected to the prep area leading to outside receptacles. Noted approximately 12 small live flies in the dining area underneath the coffe prep station (open cabinet states). Additional pest control service is needed to eliminate the pest activity. Priority foundation violation #7-38-020(a). Citation issued.
49. Non-food/food contact surfaces clean
- comments: observed accumulated food debris on interior surfaces of 2-door cooling unit with cold hold attached. Instructed to clean and maintain equipment.
51. Plumbing installed; proper backflow devices
- comments: observed high-temperature dish machine piping leaking and pooling onto the floor under equipment in the dishwashing area. Instructed to repair and maintain equipment to prevent pest harborage in said area.
55. Physical facilities installed, maintained & clean
- comments: observed dried food debris on floors under all equipment in corners of prep area, inside walk-in cooler under shelving units, and inside dry storage. Instructed to clean and maintain

- comments: observed ventilation fans in the prep area ceiling in need of cleaning to remove dust and dirt buildup
