2. City of chicago food service sanitation certificate
- comments: no original city of chicago food sanitation certificate was posted on the premises. Management instructed that a certified food manager must be present at all times with an original city of chicago food sanitation certificate while tcs foods are being prepared or served. Priority foundation violation 7-38-012.

38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises. Observed approx. 210 rodent mouse droppings in the following areas: 10 droppings on the floor of the lunchroom in the corners, 10 droppings in the old dish washing room now used as dry storage on the rolling cart, on the floor under the heat vent and on top of the heat vent, 20 droppings on the floor between the back of the walk-in cooler and the wall, 5 droppings on the floor of the cafeteria manager's office, 20 droppings beneath the sink cabinet in room 201, 5 droppings on the shelves and cabinets in room 206, 35 droppings in the main office vault beneath the sink cabinet, on the floor in the assistant principal's office and in the secretary's office, 10 droppings on the bookshelves in the media room 103, 5 droppings on the window ledge in room 101, and over 100 droppings on the shelves and in cardboard boxes in the office in the back corner of media room 103. Additional pest control service is needed to eliminate the pest activity. Management instructed to clean and remove all rodent droppings from all affected areas. Priority foundation violation 7-38-020.
