- comments: noted open baits scattered throughout the rear dry storage area along wall base. Instructed management that all pest control baits must be inclosed. Priority foundation violation #7-38-015. Citation issued.

- comments: noted over 75 mice droppings throughout the dry storage area along wall base. Instructed to detail clean and sanitize affected areas, eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority foundation #7-38-020(a).
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: noted wash rooms on premise with garbage receptacles and no lids. Instructed to provide trash cans with lids at all women wash rooms and unisex wash rooms

- comments: instructed to detail clean and maintain floors along wall base and underneath the bars and especially at the dry storage areas where mice droppings and open baits were found. Must also seal multiple holes and opeings along wall base to prevent entry places for pest.

- comments: noted missing light bulb at the rear dry storage area by the office. Instructed to provide.
