- comments: observed no handwashing soap at the handwashing sink in the rear food prep/dishwashing area. Handwasing soap provided at mentioned handwashing sink. Priority foundation 7-38-030(c), citation issued

- comments: observed no hand drying provision in toilet room. Instructed to provide and maintain. Paper towel was provided at this time. Priority foundation 7-38-030 (c), citation consolidated with above violation
38. Insects, rodents, & animals not present
- comments: observed approximately 12 or more small flies and approximately 5 rat droppings on site. Small flies were observed in the rear food prep/ dishwwashing area. The rat droppings were found on the floors near the freezer chest, on the lower shelf under the food prep table near the bin of white onions in the rear food prep/dishwashing area. Also, found rat droppings on the floor under the far-right side under the serving counter near the two door refrigerator in the front food prep/service area. Instructed to elimiate small fly activity, remove rat droppings, and clean and sanitize all affected areas. Priority foundation 7-38-020(a), citation issued

- comments: observed the right side of the freezer in the rear food prep area with build up of dried liquid food splatters. Instructed to deep clean and maintain.

- comments: observed the walls near the freezer chest in the rear food prep with dried food splatters and in need of cleaning. Instructed to clean and maintain mentioned wall.
