- comments: observed incomplete vomit and diarrhea clean up kit. Instructed to maintain a kit with all necessary supplies, a sanitizer rated as effective against norovirus and a policy. Priority foundation violation 7-38-005, no citation issued. Priority foundation 7-38-005.

- comments: 2-door prep cooler (along cookline) door rubber gasket ripped. Must repair and maintain said door gasket.
49. Non-food/food contact surfaces clean
- comments: the following not clean- observed accumulated food debris inside prep coolers lower and upper sides and ventilation hood filters with grease build up. Must clean and maintain equipment.
51. Plumbing installed; proper backflow devices
- comments: backflow prevention device not located on ice machine water line. Must provide and make visible a backflow prevention device for ice machine.
55. Physical facilities installed, maintained & clean
- comments: observed accumulated food debris on walls throughout prep and dishwash area. Must clean walls and maintain.

- comments: the following previous core violation from report #2311516 dated on 08/29/19 not corrected- #58 observed incomplete allergen training. Instructed all employees with city of chicago sanitation license must complete allergen training and keep a record on premises. Priority foundation 7-42-090. No citation issued.
