- comments: observed an employee beverage uncovered, in the food prep area. Reviewed employees beverages must be covered, with a straw and stored away from food prep areas.

- comments: bulk spice containers at the front food prep area must be labeled with the product common name.

- comments: no current pest control log book or service report on the premises. Last service report available for reviewed dated 11-30-19. Must provide a current service and service report. Priority foundation violation.

- comments: note all food handlers must wear effective hair restraints.

- comments: noted the following equipment dirty with food debris: interior front prep unused cooler, interior exterior stove, interior/exterior all coolers and freezers. Side of the pizza oven next to the deep fryer. Must clean and maintain all.

Must clean and maintain all. ----must not use paper as liner for shelving in the rear food prep area. Shelving must be smooth and cleanable.

- comments: noted an employees personal bag stored in the front food prep area. Must provide a designated storage area for employees personal belongings away from food and food contact items.

- comments: previous core violations from 2-14-20 report #2362384 not corrected. #49- observed food build-up in unused prep area 3-compartment sink and on exterior dish machine and dish machine drainboard. Must clean and maintain equipment. #51- no stoppers (x3) for rear 3-compartment sink. Must provide stoppers for a 3-compartment sink. #53- no covered trash receptacle in women's washroom. Must provide. #57- no food handlers certificates on-premises. Instructed manager all food handlers must be certified with names of employees on-premises. Priority foundation violation.
