- comments: 2-501. 11: observed all necessary items not on premises for complying with procedure available on site when employees respond to vomitting or diarrheal events. Instructed to maintain on premises all necessary items for complying with written procedure when employees respond to vomitting or diarrheal events. 10. Adequate handwashing sinks properly supplied and accessible
- comments: 5-202. 12(e): observed improper maximum water temperatures ranging between 127. 0-145. 0f at multiple handwashing sinks in student washrooms. Observed improper maximum water temperatures in both basement student washrooms, second floor boy's and girl's student washroom and boy's third floor student washroom. Instructed a handwashing sink shall be equipped to provide water at a maximum temperature of 120f through a mixing valve or combination faucet. Priority foundation violation 7-38-030(c)no citation issued.

- comments: 6-202. 15: observed openings at base of right and middle doors of triple exit doors in basement outside of both prep areas. Instructed to seal all outer doors to prevent pest entrance.

- comments: 5-204. 12: unable to locate backflow prevention device on utility sink in basement dry storage room. Instructed to install backflow prevention device on utility sink in basement dry storage room so that it shall be located so that it may be serviced and maintained.

Instructed to smoothly repair damaged walls, holes in wall, damaged floor tiles and damaged and missing baseboards for ease of cleanability. Observed cracked and peeling ceiling paint in eastside student lunchroom. Instructed to remove all cracked and peeling ceiling paint in eastside student lunchroom, instructed to smoothly repaint ceiling in eastside student lunchroom for ease of cleanability. Observed damaged floor in rear of serving line in prep area. Instructed to smoothly repair damaged floor in rear of serving line in prep area for ease of cleanability and maintain. Observed peeling paint above hood in prep area. Instructed to remove peeling paint and smoothly repaint said area for ease of cleanability above hood in prep area and maintain. Observed peeling tape on pipes throughout prep/dishwashing area. Instructed to remove all peeling paint on pipes throughout prep/dishwashing area and maintain. Observed damaged and missing tiles on wall near three door refrigerator in prep/dishwashing area. Instructed to smoothly repair or replace all damaged and missing tiles on wall near three door refrigerator for ease of cleanability and maintain.

- comments: 6-501. 14: observed dust on interior of hood above two compartment sink in prep/dishwashing area. Instructed to clean interior of hood above two compartment sink in said area and maintain.
60. Previous core violation corrected
- comments: 8-404. 13(b:4): observed previous core violation from inspection report #2613431 dated 03/10/2025 not corrected. Instructed to correct. Priority foundation violation 7-42-090 no citation issued obserrved peeling paint on walls and ceiling of boys and girls third floor restrooms. Instructed person in charge to repair all areas to be smooth and easily cleanable.
