2. City of chicago food service sanitation certificate
- comments: noted no certified food manager on duty with the city of chicago food service certificate on site while tcs foods (rice, chicken soups etc. ) were being prepared and served. Instructed management that a certified food manager with the city of chicago original food service certificate must be present at all times tcs foods are prepared and served, and also the person in charge must have a city of chicago certificate by next inspection (upon being inspected) priority foundation violation 7-38-012. Citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: management unable to provide a procedure for cleaning up vomiting and diarrhea. Instructed to have one including the chemicals and supplies needed. Priority foundation #7-38-005. Citation issued
16. Food-contact surfaces: cleaned & sanitized
- comments: noted employee washing dishes at the 3 compartment sink without setting it up to wash rinse and sanitize as required by the code. No sanitizer was used to set up the sink. Instructed management to always set up the sink when washing equipment. Manager immediately had employee set up the 3 compartment sink to wash rinse and sanitize using the chlorine. Priority violation #7-38-025. Citation issued.

- comments: noted temperature controlled for safety food items inside walk in cooler and refrigeration units not properly labeled. Instructed to properly label all ready to eat food items with product name and used by date. Priority foundation #7-38-005(a). Citation issued today.

- comments: noted establishment serving undercooked food items steak with no consumer advisory, disclosure or reminders to the customers. Instructed to have a consumer advisory on the menu with a disclosure notice and reminder statement. Priority foundation #7-38-005. No citation issued today.

- comments: noted bulk food items stored inside containers at the basement food prep/dry storage not properly labeled. Instructed management that all bulk and working food ingredient containers must be labeled with contents name.

- comments: noted food containers stored on floor in prep, storage and walk-in cooler/freezers. Must provide proper, raised shelving to store food containers 6" off floor.

- comments: noted employees at the prep area preparing food with no hair restraints or protection. Instructed to wear hair restraints or protection at all times

- comments: noted serving plates on shelves at the kitchen prep area not properly stored. Instructed to invert all serving plates to prevent dust accumulation.

- comments: instructed to detail and maintain inside service prep coolers and large mixer with dirt, spills, flour and debris.

- comments: instructed to clean and maintain floors along wall base and underneath equipment especially along wall base and underneath equipment.

- comments: noted the following previous core violation #58 rule #pa-100-0367 from inspection report #2590941 dated 03/13/2024 not corrected. Noted certified food managers on duty with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification. Priority foundation violation #7-42-090. Citation issued.
