- comments: observed male food handler drinking water from a glass in rear prep area must provide a lid and straw to prevent cross contamination. (cos)

- comments: observed poor hygienic practices by several food handlers and dish washer. Observed dish washer handling dirty dishes and then removing clean dishes from dish machine without washing hands. Also,male and female food handlers talking on cell phone with plastic gloves on and then cutting ready to eat foods,moving garbage can with gloves on and then handling raw foods. - comments: must repair or replace slow drain at hand sink on 2nd fl toilet room.

Raw chicken-47. 3f inside of 2- door reach in cooler. Product was discarded by manager. Must have cold holding foods at 41. 0f or below. Appx. 116 lbs. $150. - comments: must label food storage containers (squeeze bottles,shakers,pans,bulk) when food is not in original package.

- comments: observed evidence of appx. 100 or more mice droppings on floor underneath furnace in 1st fl. Kitchen,inside both fryer cabinets,on floor next to dish storage rack,chemical storage room,music room,on ledge under front service counter,basement storage area-on & behind paper storage shelves,tool storage,soda tanks,electrical panels and motor of unit2nd fl. Bar area- ledge and next to furnace. Also. Evidence of appx. 5-7 live small flies at hand sink in front prep area hand sink and several dead water bugs on floor under dish machine. Must remove mice droppings and water bugs clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 6/6/25. - comments: must invert multi-use utensils on dish storage shelves throughout.
