- comments: observed no city of chicago certified food service sanitation manager on duty at the time of inspection while time temperature and control for safety foods are stored, prepared and/or served. Insturcted a certified manager with a current/valid city of chicago certificate must be on premises during operation hours. Priority foundation violation. 7-38-012. Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed the ice bin interior in need of cleaning. Instructed to remove ice, clean and sanitize. Observed improper storage of ice scoop directly inside the ice machine in the ice. Instructed to store properly between uses.
38. Insects, rodents, & animals not present
- comments: observed pest control logbook with no current service reports since 4/23/24. Instructed to provide. Observed pest activity with approx. 15-20 small gnats/flies in the rear of the facility at the utility sink area noted during inspection. Instructed to consult pest control technician for service. Priority foundation violation. 7-38-020(c). Citation issued.
51. Plumbing installed; proper backflow devices
- comments: observed drainpipe leaking under hand washing sink in the rear dishwash area. Instructed to repair.
55. Physical facilities installed, maintained & clean
- comments: observed mop stored improperly in rear of facility at the utility sink. Instructed to store properly with mophead in the upright position.
57. All food employees have food handler training
- comments: observed no proof of training for all food handlers. Instructed to provide.
58. Allergen training as required
- comments: observed no food employee with proof of allergen training. Instructed to provide for all city of chicago food service sanitation managers.
