5. Procedures for responding to vomiting and diarrheal events
- comments: found no procedure/plan and kit for responding to vomiting and diarrheal events. Instructed facility to develop and maintain a procedure/plan and to maintain any appropriate supplies on site. Priority foundation violation 7-38-005. No citation issued

38. Insects, rodents, & animals not present
- comments: we observed evidence of pest activity in different places of premises. Mice droppings were observed in the kitchen:30 or more droppings under and on top shelves,beside cooking equipment, 10 or more droppings at front window ledges. Rear storage area over 60 droppings under and behind cooler units. Instructed to clean,sanitize and maintain areas. Priority foundation violation:7-38-020(a),citation issued

- comments: must not use milk crates as a means of elevation for stored items.

- comments: 4-601. 11(b) must not use cardboard as liner for shelving. Surface must be smooth and cleanable

51. Plumbing installed; proper backflow devices
- comments: not able to locate a back flow prevention device at dispenser unit behind three compartment sink where water and soap is dispensed. A black hose is attached from unit to water line of faucet. Must install so back flow prevention device may be located to be serviced and maintain. If unit is not in use remove said unit

- comments: lid is needed at receptacle inside the washroom.

- comments: observed expanding spray foam used as a sealant for openings around floor piping behind shelves throughout the premises. Must remove to be smooth and cleanable.
