- comments: observed one small mouse dead inside of a trap in the rear boiler room area. Instructed to remove.

- comments: observed the sides of the cooking equipment and non cooking equipment throughout facility with a build up of grease, dirt, dust and food debris. Instructed to clean and maintain all as a means of preventing possible pest and insect activity.
55. Physical facilities installed, maintained & clean
- comments: observed the floor and walls under and around the cooking equipment and the three compartment sink with a heavy build up of grease and food splatter as well as the pipes behind the equipment. Instructed to clean and maintain all areas.
56. Adequate ventilation & lighting; designated areas used
- comments: observed the hood ventilation and filters with a build up of grease. Instructed to clean and maintain as a means of preventing possible contamination of foods cooked in the area.
