- comments: observed improper temperature of tcs ready to eat foods inside of 2-door prep cooler, such as 2 lbs. Cut lettuce & tomato bed-61. 3f. 3lbs. Baked potato skins-58. 1f. . 5lb. Pico de gallo-62. 1f. 2gallons ranch dressing-57. 0f-58. 3. 1-gallon blue cheese dressing-59. 3f-66. 2f. The product was discarded by the manager. Must have cold holding foods at 41. 0�f or below. - comments: observed menu containing items served undercooked, such as hamburgers and ceviche, without disclosure, but containing reminder and disclaimer. Instructed to mark all raw or undercooked items with a disclosure device that links to reminder and disclosure. Priority foundation violation 7-38-005. No citation issued.

- comments: observed improper temperature of 2-door prep cooler at 55. 4�f with tcs ready to eat foods stored inside packed with ice inside bins. A held for inspection tag placed on the unit. - comments: must label food storage containers when food is not in original package. (squeeze bottles)

- comments: observed evidence of appx. 75 or more rat dropping in bar area under ice bin cooler & table, in basement on floor & shelving units in basement liquor storage area and on other side of personal & business storage area. Must remove droppings, clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/6/25. (priority foundation 7-38-020(a)(citation issued)

- comments: must provide a splash guard at hand sink next to 3- compartment drain board in main prep area.
