1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed person in charge with no valid proof of food safety management training.

2. City of chicago food service sanitation certificate
- comments: observed facility food handlers preparing and handling time and temperature control for safety (tcs) foods without a city of chicago food service manager certificate holder on site. A certified food manager arrived at 1:25. Informed person in charge a certified manager shall be on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.

6. Proper eating, tasting, drinking, or tobacco use
- comments: observed food handler store drink on pizza tray tabletop. Instructed to store and consume food and beverage in desginated area.

10. Adequate handwashing sinks properly supplied and accessible
- comments: noted no handwash signage at all handsinks in kitchen/ prep areas. Instructed to provide and maintain.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed high temperature warewashing machine not meeting hot water sanitizer requirements of at least 180�f for the final rinse temperature. Recorded temperature 69. 5 c (157. 1 f) machine tagged priority violation #7-38-025. Citation issued.

37. Food properly labeled; original container
- comments: noted bulk dry ingredients in white bins in kitchen without labels. Instructed to provide labels of common names of items removed from its original container.

40. Personal cleanliness
- comments: observed food handlers in kitchen without proper head coverings. Instructed to provide and maintain. Proper hair restraints

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: noted no covered receptacles in downstairs womens washroom. Instructed to provide and maintain.

58. Allergen training as required
- comments: observed no proof of allergen training for all certified foodservice managers. Instructed all employees holding a city of chicago foodservice manager certificate to comply and complete allergen training and maintain records on site at all times.
