Report date: Jan 15, 202638. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises. Observed approx. 15 rat droppings on the floor beneath the mop sink and beneath the dry storage shelving next to the rear exit door. Also observed approx. 30 mouse droppings on the floor of the rear food prep area under the dry storage shelves, in the wooden cabinets behind the front counter and behind the 2-door prep cooler against the wall behind the front counter. Management instructed to clean and remove all rodent droppings throughout. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed dead cockroaches in the drawers and cabinets behind the front counter. Management instructed to clean and remove all dead insects from the premises.
49. Non-food/food contact surfaces clean
- comments: clean the exterior of the grease trap located beneath the 3-compartment sink. Clean all cooking equipment with encrusted grease buildup including the oven, deep fryers and stoves. Clean the stainless steel storage shelves at the cook's line and exterior of all food storage containers.
51. Plumbing installed; proper backflow devices
- comments: close and seal the lid for the grease trap to prevent pest activity.
55. Physical facilities installed, maintained & clean
- comments: clean the grease and food debris on the floor beneath the dry storage shelves and under the cooking equipment. Clean the grease on the wall behind the cooking equipment.
