- comments: observed no certified manager on site while the facility was open and operating. Instructed to maintain an employee holding valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no thermometer inside prep cooler used to store heavy cream. Instructed to provide.

- comments: the observed key is stuck in the door to 1 of 2 unisex toilet rooms. Instructed to correct and maintain in good condition.

- comments: observed hose used to drain the air conditioning unit runs out of the ceiling and into the exposed handwashing sink located next ice machine. Instructed to correct and maintain in good condition.

- comments: observed several holes in the ceiling above the ice machine. Instructed to repair or replace and maintain in good condition.

- comments: observed build-up of dust on vent above utility sink. Instructed to detail clean and maintain in good condition.
