2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on the premises while tcs foods (lamb, chicken, salmon) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.

22. Proper cold holding temperatures
- comments: found cold tcs foods at improper cold holding temperatures inside the 2-drawer cooler located on the cooks line. Beef patties, turkey patties, lamb chops, and salmon found at temperatures ranging from 57f to 60f. Instructed to discard and denature said tcs foods and always maintain all cold-holding tcs foods at or below 41f. Priority violation 7-38-005. Citation issued. Food discarded.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: equipment not maintaining proper temperature. Noted temperature of the 2-drawer cooler at the cooks line to be improper at 46. 0 f. Unit tagged and held for inspection by the chicago department of public health and may not be used until re-inspected by cdph and maintaining proper temperature of 41f and below. Priority violation 7-38-005. Citation issued.

43. In-use utensils: properly stored
- comments: found ice scoop being stored inside ice machine with handle in ice. Must store ice scoop in a clean location protected from sources of contamination or inside machine with scoop handle out of ice.

49. Non-food/food contact surfaces clean
- comments: observed heavy food debris and grease accumulation inside the coolers on the cooks' line. Must detail clean inside coolers and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed the dish area handwashing sinks faucet leaking. Must repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed lid needed to garbage can inside the unisex (left) washroom. Instructed to provide.

56. Adequate ventilation & lighting; designated areas used
- comments: observed heavy dust build up on the ventilation fan covers inside the washrooms, above the dish area, and the bar. Must clean all and maintain.
