- comments: 6-501. 111: observed approximately 10 live roaches in facility, observed live roaches crawling inside employee lockers and on floor in employee locker room in basement. Instructed to eliminate roach activity in facility and clean affected areas, recommended pest control service facility on or after 12/10/2025 to eliminate roach activity. Priority foundation violation. 7-38-020(a) citation issued on 12/10/25

- comments: 6-501. 112: observed dead roaches throughout locker room. Observed dead roaches on floors, in corners and in lockers of locker room. Instructed to remove all dead roaches in facility and clean affected areas.

39. Contamination prevented during food preparation, storage & display
- comments: 3-305. 11: observed no ice bin cover/lid for ice in ice bin in interior of bar in basement. Instructed to provide cover/lid for ice bin in interior of bar in basement to prevent exposure to splashes, dust or other contamination of ice in ice bin during storage.

39. Contamination prevented during food preparation, storage & display
- comments: 3-307. 11: observed no left splash guards on handwashing sinks in first floor dishwashing area and at handwashing sink in first floor dining area across from bar where pitchers of water are stored(beverage station near hallway exit door). Instructed to install left splash guards on handwashing sinks in first floor dishwashing area and at handwashing sink in first floor dining area across from bar where pitchers are stored to prevent contamination of clean equipment in dishwashing area and pitchers of water in beverage station across from first floor bar(near hallway exit door).

- comments: 5-205. 15: observed drain pipe underneath ice machine in basement leaking into pan on floor. Instructed to repair leak from drain pipe of ice machine in basement.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 11: observed missing grout in between floor tiles in basement prep area(near 1 comp sink and handwashing sink), in first floor prep area in cookline area and near floor trash shoot in first floor prep area. Instructed to smoothly regrout in between floors tiles where grout is missing in basement prep area(near 1 comp sink and handwashing sink), in first floor prep area in cookline area and near floor trash shoot in first floor prep area. Observed damaged floor tiles in walk in keg cooler in basement. Instructed to smoothly repair or replace damaged floor tiles in walk in keg cooler in basement for ease of cleanability.

- comments: 6-501. 12: observed ice buildup and mounds of ice on floors underneath shelving units and ice buildup on ceiling vent in basement walk in freezer. Instructed to remove ice uildup and mounds of ice from floors underneath shelving units and ice buildup on ceiling vent in basement walk in cooler and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: 6-303. 11: observed non working lights in employee locker room and in hoods of first floor prep area. Instructed to repair or replace all non working lights in employee locker room and in hoods of first floor prep area to increase light intensity.
