- comments: observed no handwashing signage in toilet room on 2nd floor. Instructed manager to provide hand washing singnage.

- comments: observed more than 50 flies throghout facility including in kithcen area. Instructed manager to service all areas affected by pests. Violation consolidated with violation #38 on report #2624599 priority foundation violation 7-38-020(a) citation issued.

- comments: observed dirty gasket on walk in cooler on 2nd floor. Instructed manager to clean.

- comments: observed hand washing sink to be slow draining and have a leak at bottom of sink in 2nd floor kitchen. Instructed maanger to repair hand washing sink.
