5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit and diarrhea, clean up kit and no corresponding written procedure on responding to vomit and diarrhea events. Instructed facility to provide and maintain appropriate supplies and develop a clean-up procedure. Must keep both the policy and supplies on site at all times. Priority foundation violation #7-38-005. Citation issued.

6. Proper eating, tasting, drinking, or tobacco use
- comments: observed employee smoking at rear exit door. Noted ash tray on outdoor break table. Instructed not to smoke within 15ft of establishment entryways

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located in basement with a build-up of black and brown substance, dripping onto ice cubes stored below the ice bin. Instructed to clean, sanitize, and maintain. Priority foundation violation #7-38-005. Citation issued

38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises. Observed approximately 100 mouse droppings on the floor in basement in various locations: under dry food and supply storage racks, on wooden pallets and in metal food storage containers stored on a pallet in the southeast corner of the basement, and between the ceiling and drop ceiling attached to the stairway leading to the basement. Management instructed to remove all droppings, clean and sanitize all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation #7-38-020(a). Citation issued.
