2. City of chicago food service sanitation certificate
- comments: 2-101. 11 observed facility food handlers preparing and handling time and temperature control for safety (tcs) foods without a city of chicago food service manager certificate holder on site. A certified food manager arrived at 2:10 pm. Business opened at 11 am. Informed person in charge a certified manager shall be on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.
59. Previous priority foundation violation corrected
- comments: 8-404. 13 (b:3) observed previous priority foundation violations not corrected within the specified time frame on the inspection report#2621521 dated for 7/30/2025: (5) observed no vomit and diarrhea, clean up kit and no corresponding written procedure on responding to vomit and diarrhea events. Instructed facility to provide and maintain appropriate supplies and develop a clean-up procedure. Must keep both the policy and supplies on site at all times. Priority foundation violation (16) observed ice machine located in basement with a build-up of black and brown substance, dripping onto ice cubes stored below the ice bin. Instructed to clean, sanitize, and maintain. (38) observed evidence of rodent activity on the premises. Observed approximately 100 mouse droppings on the floor in basement in various locations: under dry food and supply storage racks, on wooden pallets and in metal food storage containers stored on a pallet in the southeast corner of the basement, and between the ceiling and drop ceiling attached to the stairway leading to the basement. Management instructed to remove all droppings, clean and sanitize all affected areas. Additional pest control service is needed to eliminate the pest activity. Instructed person in charge that the violation shall now be classified as a priority item due to previous noncompliance. Must correct before re-inspection. Priority violation #7-42-090. Citation issued
