2. City of chicago food service sanitation certificate
- comments: observed manager william klein on site without the city of chicago foodservice manager certification. Inform that a manager with the certification needs to be on site from open to close while tcs foods (chicken, cole slaw) and other items are being prepared and served. Instructed mr klein to obtain the city of chicago foodservice manager certification and provide on site (proof of payment for certification will be accepted) priority foundation violation 7-38-012 citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed a build up of grease and dust under the heat lamp area where the cooked chicken is held for service. Instructed to clean and maintain as a means of preventing possible contamination of foods held in area.

- comments: observed a heavy build up of grease between the deep fryer stations in the front food service area. Instructed to clean and maintain as a means of preventing possible pest and or insect activity.

- comments: observed the walk in cooling unit and walk in freezer with a build up of dirt and debris on the floor under the shelving units. Instructed to clean and maintain areas.

- comments: observed the floors in throughout the facility in need of a detail cleaning. . . The rear two compartment sink area. . . The three compartment sink area. . . The soda holding rack across from the office and the front food prep and service areas. Instructed to clean and maintain all areas.

- comments: observed no allergen training certificate on site for manager william klein. . . Instructed to provide and maintain as required or citations will be issued.

59. Previous priority foundation violation corrected
- comments: previous priority foundation violation not corrected from report #2629158, date 12/30/25. Still observed numerous to many to count live roaches at the front food service counter behind and around black boxes under the lemonade machine (soda compresser unit). Also noted live roaches on the wall above the 3 comp sink, wall under cashier register, behind rear ice machine and rear washing area. Violation codes: 6-501. 111 inspector comments: observed evidence of live roach activity in the rear dish washing area approx 10 live roaches. . . The dish storage rack approx 2 live roaches. . . And under the sink at the pipes approx 8 live roaches. . . The front food service area the black boxes right above the outlets under the counter where the lemonade machine is located. . . Instructed to have pest inspection and service before return date of 01/06/2026. Must remove and detail clean all affected areas. Also call pest control for service.
