- comments: 6-501. 111 observed approx. 35 live small flying insects on the walls and ceiling of the washrooms, food prep area and dining room area next to the fountain pop dispenser. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: 6-501. 19 observed no self-closing door devices provided for the washroom doors. Management instructed to install.

- comments: 8-404. 13(b)(3) previous priority foundation violation from report #2594425 on 5/10/24 not corrected: 54 - observed the lid of the grease dumpster in the alley removed and the grease dumpster filled with grease and garbage overflowing over the top of the dumpster. Also observed excessive amount of garbage and litter on the ground around the dumpsters. Management instructed to have the grease dumpster emptied and clean the ground surrounding the dumpsters. Priority violation 7-42-090. Citation issued.
