- comments: 2-501. 11 observed no written procedure nor supplies to respond to vomit and diarrhea clean up. Instructed to provide. Priority foundation. Citation issued 7-38-005.

- comments: 6-301. 12 observed no hand drying provision located at the hand washing sink in the food prep area. Instructed to always maintain a hand drying provision at all hand washing sinks at all times. Priority foundation. Citation issued 7-38-030(c).

- comments: 6-501. 111 observed evidence of rodent activity. Observed about 30 rodent droppings located in various areas of the food prep areas. Observed about 20 rodent droppings behind the stairs that lead to the basement. Instructed to clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-501. 114 observed excessive unecessary clutter located thru out the rear of the facility and stored on the floor. Instructed to remove unecessary clutter and elevate all items at least 6 inches or above and maintain.

Instructed to clean inside of all refrigeration equipment and maintain. Observed accumulated grease on the vents located above the cooking equipment. Observed accumulated food debris on the handles of all the refrigeration units and cooking equipment. Instructed to clean and sanitize all.

- comments: 6-303. 11 observed burnt out lighting in the hallway and rear food prep area. Instructed to replace light bulbs and provide adequate lighting through out the rear and hallway and maintain.

- comments: 8-404. 13(b:4) observed previous minor (core) violations not corrected from inspection report #2282604 dated 4/4/2019; 47-observed many food items pre cut improperly stored in grocery bags in coolers, and freezers. Instructed to store in food grade containers, or food bagies/ziploc bags. 49-observed the cooking equipment not clean with food debris, and grease, instructed to clean interior and exterior of all cooking equipment. 55-observed the walls in the rear prep area not clean, instructed to clean remove food debris, also clean front prep area ceiling. 58-observed no alergen training certificates on premises at this time, instructed to provide. Instructed to correct all previous minor (core) violations. Priority foundation. Citation issued 7-42-090.
