- comments: observed no certified manager on site at the time of inspection while tcs items such as chicken were prepared. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towels at both exposed handwashing sinks. Instructed to provide paper towels at all handwashing sinks at all times. Priority foundation violation 7-38-030(c). Citation issued.
21. Proper hot holding temperatures
- comments: observed chicken in hot hold unit in prep area held at improper temperature of 125. 9f. Management discarded approximately 5 lbs of product valued at approximately $30. Instructed to maintain all tcs items at or above 135f at all times. Priority violation 7-38-005. Citation issued.

- comments: observed 2 female and 1 male food employee working without hair restraints. Instructed to provide.

- comments: observed torn gaskets on the prep cooler and prep freezer. Instructed to repair or replace and maintain in good condition.

- comments: observed no sink stoppers for 3 compartment sink. Instructed to provide.

- comments: observed urinal in men's toilet room containing toilet as well does not flush. Instructed to repair or replace and maintain in good condition.

- comments: unable to locate grease trap connected to 3-compartment sink. Instructed to install by all relevant laws, rules, and regulations.

- comments: observed no covered trash receptacle inside women's toilet room. Instructed to provide.
57. All food employees have food handler training
- comments: observed 2 female employees working without food handler training. Instructed all food employees must complete food handler training and maintain records thereof on site at all times.
