- High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 1) observed approximately 28 flying insects at dish washing area. 2) observed approximately 40 flying insects at mop sink area. 3) observed approximately 15 flying insects in bagless trash can near walk-in cooler. 4) observed approximately 5 flying insects landing on clean dishes near fountain drink machine. 5) observed approximately 12 flying insects in dry storage area landing on rice, beans, and cooking oil containers. **Warning**
- High Priority - Presence of insects, rodents, or other pests. Observed approximately two spiders on wall above back door feeding on flying insects. **Warning**
- High Priority - Raw animal food stored over or with unwashed produce. Raw shell eggs above unwashed produce in walk-in cooler. Discussed with person in charge. Person in charge placed eggs on bottom shelf. **Corrective Action Taken**
- High Priority - Roach activity present as evidenced by live roaches found. 1) Observed 1 live roach on floor near chemical storage rack in back of kitchen. 2) observed 1 live roach on floor in dry storage area. 3) observed 2 live roach on wall by back door next to reach in cooler. **Warning**
- High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. slaw (58F - Cold Holding) held in glass door reach in cooler that person in charge states is not functioning correctly. Slaw kept in reach in cooler for 1 hour. Person in charge moved slaw to walk-in cooler to cool. No other foods inside glass door reach in cooler. **Corrective Action Taken**
- High Priority - Toxic substance/chemical improperly stored. Observed Sprayed bottles stored at bar near drink mixers. Person in charge removed chemicals spray bottles from food. **Corrected On-Site**
- High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed Hose Bibb in used near back door missing vacuum breaker.
