- comments: no person in charge with foodservice sanitation training.
2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago sanitation certificate on site at time of inspection. Instructed person in charge that valid certificate as well as certified manager must be on site at all times. Priority foundation 7-38-012. Citation issued.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in unisex restroom for female sanitary products disposal. Instructed person in charge to provide covered receptacle and maintain at all times.
55. Physical facilities installed, maintained & clean
- comments: observed peeling paint on wall behind chest freezer (closest to exterior door). Instructed person in charge to repair to be smooth and easily cleanable.
60. Previous core violation corrected
- comments: observed following core violation from report #2574822, dated: 4/24/23 not corrected; #55 observed small hole in wall behind chest deep freezer near utlity panel. Observed broken floor tile in rear dry storage area by hot water tank near ramp. Instructed person in charge to repair and maintain. Priority foundation 7-42-090. Citation issued.
