- comments: observed no city of chicago certified food manager on site during time of inspection with valid original certificate while preparing and serving tcs foods (milk). Instructed to provide manager on-site all times while preparing food and being open and operating. Priority foundation violation. #7-38-012. Citation issued.

- comments: observed no hand drying provision at multiple handsinks (prep and 2 out of 3 classroom handsinks) during the time of inspection. Instructed to provide and maintain at all times while open and operating for proper hand washing. Priority foundation violation. #7-38-030(c). Citation issued.

- comments: ibstructed to provide thermometers for inside of cooling units.

- comments: observed approximately 22 small rodent droppings on the floor inside of the dry storage closet located inside the prep area. Recommended to contact pest control for service, remove droppings and sanitize area. Priority foundation violation. #7-38-020 (a). Citation issued.

- comments: observed items stored on floor inside storage closet in prep area. Instructed to organize said space, remove crates and properly elevate all items at least 6 inches off floor.
