- comments: observed approximately 150 rat droppings on the floor in the basement liquor storage rooms along the walls and pallets. Management instructed to clean and remove the droppings from all affected areas. Also observed 10 live small flying insects behind the walk-in bar. Additional pest control service is needed to eliminate all pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: clean the exposed hand washing sink and the ice bin behind the walk-in bar.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: the washroom in the liquor store lacks a self-closing door device. Management instructed to install.

- comments: observed no written record on the premises describing the protocol for checking proof of vaccination in public places. Must provie and maintain.
