- comments: observed no written employee health policy on the premises. Instructed to provide a signed employee health policy for each employee. Priority foundation violation 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain the required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops, and appropriate sanitizer/spray that is effective against norovirus). Priority foundation violation 7-38-005.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hot running water at the handwashing sinks in the men's and women's washrooms. Water temperatures ranging from (65. 7f-70. 0f). Instructed management handwashing sinks water temperature must be provided at least 100 f through a mixing valve or combination faucet. Instructed management to repair and maintain as required. Priority violation 7-38-030(c). Citation issued.

- comments: found no licensed pest control logbook available on premises. Instructed to provide a pest control logbook with a complete contents (contract/service agreement, service report, chemicals used, copy of the idph license, etc. ) priority foundation violation 7-38-020 (c), citation issued

- comments: instructed to properly store in a clean separate container to prevent contamination before use handles-up.

- comments: instructed to provide tight fitting lid/cover for ice bin behind the bar.

- comments: observed no chemical test strips on the premises to measure the sanitizer concentration at the 3-compartment sinks. Instructed to provide and maintain. Priority foundation violation 7-38-005,citation issued.

- comments: observed water leak at drainpipe at handwashing sink located on the west end of the bar area. Instructed to repair plumbing leak and maintain.
