- comments: observed one hundred live small flies on walls in dishwash room, also observed 50 live small flies on walls in prep area. Instructed manager all small flies must be removed from premises by date of re-inspection. Priority foundation 7-38-020(a). Advised to call pest operator for service.

- comments: observed accumulated food debris on prep area shelves, interior of unused coolers, interior of all sinks. Instructed manager must clean all equipment.
55. Physical facilities installed, maintained & clean
- comments: observed accumulated food and dust build-up on ceiling tiles and walls at the dishwashing prep area. Instructed manager must clean and maintain.
56. Adequate ventilation & lighting; designated areas used
- comments: observed missing protective light cover at the prep and dishwashing area. Instructed to retrofit with light covers and maintain.
