- comments: observed evidence of pest activity on site during the time of inspection. Noted approximately 125 small live flies flying throughout prep areas in rear of facility on walls above sinks and on handsink in deli prep area, on prep/storage table surfaces, flying throughout produce stands and on exterior surfaces of stored cooking equipment. Noted approximately 12 small rodent droppings on the floor near the locked storage room and 40 live ants crawling in the same area. Instructed to contact pest control for additional service. Must eliminate pest activity on the premises. Priority foundation violation. 7-38-020 (a). Citation issued.

- comments: observed equipment in taqueria area where seating is in need of cleaning (cold hold drawers and shelves). Instructed person in charge to clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed broken tile near cold hold unit(milk/dairy). Instructed person in charge to repair and make floor smooth and easily cleanable.
